CURRENT VIEWS ON PLATELET–RICH PLASMA THERAPY AND PROSPECTS OF ITS APPLICATION IN PATIENTS WITH HEMOPHILIA WITH ARTHROPATHIES

Abstract
The article is devoted to the analysis of modern views and prospects for the use of PRP therapy (platelet-rich plasma) in the treatment of hemophiliac patients with arthropathies, which is one of the most common complications of hemophilia and leads to persistent disability. The relevance of this direction is substantiated; the limited effectiveness of traditional approaches to the treatment of joint lesions in the early stages of their formation is emphasized.
The article examines in detail the biological mechanism of action of PRP therapy, which consists in the influence of concentrated growth factors and other biologically active molecules of autologous platelets. These substances, when introduced into the affected joint, contribute to the launch of natural regenerative processes, reduce inflammation, stimulate the synthesis of extracellular matrix components and, probably, restore articular cartilage. The results of clinical studies demonstrating the therapeutic potential of PRP therapy in reducing pain, improving joint mobility and inhibiting degenerative changes are analyzed. Particular attention is paid to the safety of this method for hemophilia patients, its possible combination with traditional hemostatic therapy regimens, and the prospects for introducing PRP into clinical practice.
It is emphasized that, despite the encouraging results and significant advantages (minimally invasive, autologous origin, safety due to the use of the patient’s own blood components, high concentration of platelet-derived growth factors in the injection area, stimulation of chondrocyte synthesis of the extracellular matrix of cartilage), further large-scale and standardized studies are needed to finally confirm the long-term effectiveness and develop optimal protocols for the use of PRP therapy in this category of patients, and to understand the role of regenerative medicine in the treatment of hemophiliacs with arthropathies. Further experimental and clinical studies in these areas are undoubtedly promising and will significantly expand the clinical potential of orthobiological medicine.
