Abstract

Aim. To conduct a comparative analysis of the incidence and characteristics of the clinical course (distribution by age groups, mortality, survival, the effect of TKI therapy) on the course of CML among the adult population of the Dnipropetrovsk region, taking into account the possible long-term effects of various adverse environmental factors.

Materials and methods. Ananalysis of the incidence of chronic myeloid leukemia in the specified regions for 12 years – from 2006 to 2017.

Results. A 12-year study of the incidence of chronic myeloid leukemia (CML) inresidents of the Dnipropetrovsk region was conducted. A comparative analysis of the incidence of CML among the population of the regions of the Dnipropetrovsk region was carried out, taking into account the possible influence of various adverse environmental factors (radiation, air, water and soil pollution). Ananalysis of age groups, mortality rates, the effect of the use of tyrosinekinase inhibitors (TKIs) on the duration of treatment and survival was carried out. A difference in the distribution between age groups was revealed in comparison with literature data. Fluctuations in the incidence of CML among residents of the Dnipropetrovsk region for the period 2006-2017 and the influence of negative environmental factors that cause an increase in incidence in large industrial cities contaminated with radioactive and chemical substances have been revealed.

Conclusions. According to the results of the study, it was established that the increase in the incidence of chronic myeloid leukemia, the course of the disease, and mortality are influenced by negative environmental factors, the patient’s age, gender, the adequacy of the performed diagnosis, and the timelines of prescribing targeted therapy. Fluctuations in the incidence of CML depending on the contamination of the territory with a complex of adverse factors are characteristic of the Dnipropetrovsk region. Timely appointment of targeted therapy to patients with CML significantly increases survival, course of the disease and quality of life.
